MUB utilizes different methods for submitting forms, the majority of which utilize DocuSign, a highly secure and well-respected company used for obtaining electronic signatures. Once you have completed a DocuSign form, the document will be securely forwarded to the appropriate MUB representative and you will receive a copy of the document via e-mail. Landlord Agreement - Permanent Signature - If you manage a rental property, use this form to allow for continuous service at a property or properties. Using this form means that MUB will not disconnect service between tenants.
Discontinuation of Landlord Agreement - If you have a Landlord Agreement with us, use this form to terminate that agreement with MUB. Using this form means that MUB will disconnect service between tenants.
